Truthful Application, Potential Fees

By completing and submitting an application to Leading Providers, Applicant hereby agrees and acknowledges that all of the information submitted is accurate to the best of Applicant’s knowledge. Applicant also agrees and acknowledges that Leading Providers may charge service fees for any services rendered to Applicant. Any services and associated fees will be agreed to by Applicant in advance of provision of such services.
